Purchase of Rock Salt from Compass Minerals America, Inc., Overland Park, Kansas, for the 2019-2020 Winter Season - Change Order No. 1 and Final. Alderman Beifuss requested an explanation for the Change Order. Mr. Flatter explained that normally this is not an issue because the City has never had enough capacity to order the full allotment of rock salt in any one given year. However, with the new salt storage shed the City was able to order the full 4,600 tons this year. Each truck is weighed before delivery, but fluctuations in amounts loaded into a truck are expected and uncontrollable. Once a truck is loaded, the vendor will not unload a portion of the load if overage occurs. This resulted in an overage of 55.75 tons received in the last delivery. Mr. Flatter explained that this is a nominal amount, and it is nearly impossible to guarantee that the total amount received will not go over if the full allotment is ordered. Only two contractors responded to the City's RFP and I have attached a tabulation of pricing received. Hushion Pavement Maintenance, lnc.'s proposals, which totaled $42,059.00, were substantially lower than Midwest Sealcoat's proposals of $112,836.00. It is staff's opinion that the proposals received, and/or reason other contractors did not submit a proposal, were reflective of the performance requirements identified in the City's Request For Proposal (RFP) documents, which included: 1. Contractor must be able to complete work prior to November 1, 2020. 2. Contractor must be willing to perform services in phases, after normal business hours, and/or on weekends, if determined necessary by City. 3. Contractor must comply with City's insurance requirements. 4. Payment of prevailing wages will be required with submittal of waivers of lien and certified payroll where appropriate and applicable. The last time these parking lots received a protective layer of sealcoat was 2016. Sealcoating of parking lot(s) is a tried and proven maintenance method to help preserve the integrity, visual appeal, safety, and durability of the property. It can be very costly to replace or fix the asphalt in the parking lot after harsh winter weather, but the City can save significant dollars by remaining proactive with its maintenance programs. When sealcoating is applied to a parking lot surface, it adds a protective layer that acts as a barrier between the harsh environment's wear and tear, and the asphalt of your parking lot. Without proper maintenance, a parking lot can fall apart beyond repair, and will then need to be replaced at significant cost. Sealcoating is extremely important in the maintenance of a parking lot for many reasons, including the following: 1. Extension in the life of the parking lot; and, 2. Prevent high repair costs; and, 3. Blocks the drying effects of the sun and protects against freeze and water; and, 4. Prevent damage from auto fluids; and, 5. Keep the parking lot looking acceptable and maintained. For appearance, pavement preservation, and to help delay significant replacement costs, staff recommends awarding a contract to Hushion Pavement Maintenance, Inc., of West Chicago, Illinois, for an amount not to exceed $42,059.00, for parking lot sealcoating and striping services at various City owned facilities during fiscal year 2020. Guttman ---------~ ITEM SUMMARY: As part of the City's water treatment process, rotary hydrated lime is used to purify the raw water through a softening process. When the lime bonds with targeted minerals (i.e., radium, calcium, magnesium, etc.) sludge is generated. The sludge is transferred to three exterior holding lagoons until contractually removed. The last such project was completed in late 2018 for a total project cost of $591,242.55. For Fiscal Year 2020, within the Water Fund (06-34-48-4210), $700,000.00 has been budgeted for removal and disposal of waste lime sludge from the City's Water Treatment Plant lagoons (1400 Hawthorne Lane). All removed sludge must be disposed of in an EPA approved landfill. The project was advertised in the Daily Herald on August 4, 2020, and bids were opened on August 25, 2020. Heartland Recycling-Aurora CCDD, LLC has provided waste lime sludge removal services for the City of West Chicago in 2008, 2013, and 2018, and performed well. However, it is our understanding that the all maintenance and inspection responsibilities under the Storm Water Pollution Prevention Plan (SWPPP) will be performed by the developer's representative. This will include but is not limited to topsoil, plantings, sod, seed, rip rap, etc. Exclusions from TEG Scope of Work: Erosion and sediment control reporting, mass grading, earth excavation, and construction layout and staking are excluded from our proposed scope of services unless otherwise requested by the City. TEG's scope of services for specific work items is outlined above. Generally, TEG will provide inspection for all roadway, pavement, water main, sanitary sewer, and storm sewer installation. Material testing will be under a separate contract with the developer to provide Quality Control (QC) testing of roadway subgrade, aggregate subgrade/base course, combination concrete curb and gutter, PCC sidewalk, and HMA pavement as requested by the City. TEG will coordinate with the developer's material testing firm as necessary. The following is a listing of the scope of services presented in a logical sequence. PRE-CONSTRUCTION PHASE 1. Review the status of all permit requirements and verify contractor is in compliance prior to beginning the associated construction activity. 2. Set up field books, quantity book, diary or other forms of documentation. 3. Verify layout control if contractor has responsibility for staking, otherwise perform construction layout and staking ongoing throughout project. 4. Review submittal and record drawing requirements with contractor. 5. Check and approve, or reject and request re-submittal of, any submittals made by the contractor for compliance with the contract documents. 6. Notify the City of upcoming activities that will impact access, including, but not limited to road closures, driveway access interruption, or temporary utility shut downs. 7. Attendance of a pre-construction meeting, up to 3 meetings. CONSTRUCTION PHASE Our entire Construction staff is currently certified in IDOT's Documentation of Quantities to ensure proper inspection and documentation of the work performed. 1. Observe the progress and quality of the executed work. Determine if the work is proceeding in accordance with the Contract Documents. TEG will keep the City informed of the progress of the work, guard the City against defects and deficiencies in the work, advise the City of all observed deficiencies of the work and disapprove or reject all work failing to conform to the Contract Documents. 2. Maintain daily contact with City. Inform them of progress on the project as well as any cost or quality related issues that may develop. Also help to coordinate between the contractor and any other agency officials that may have an interest in the project. 3. Maintain daily contact with the contractor to determine work schedules, coordinate underground work affecting access with appropriate parties, determine paving and concrete placement dates, and adherence to overall project schedule. Ensure contractor is following applicable City standards and specifications during installation. b. Serve as the City liaison with the contractor working principally through the contractor's field superintendent. c. Attend all construction conferences. Arrange a schedule of progress meetings and other job conferences as required. Maintain and circulate copies of records of the meetings. d. Ensure layout is accurate and complete. e. Maintain orderly files of correspondence, reports of job conferences, shop drawings, contractor's request for information, and other submissions, reproductions or original contract documents including all addenda, change orders and additional drawings issued subsequent to the start of the contract. f. Maintain inspector's daily report book in the City's preferred format, or other required format appropriate for the project, recording hours on the job site, weather conditions, general and specific observations, daily activities, quantities placed, inspections, decisions, and list of visiting officials. g. Coordinate material testing with the City and/or contractor's quality control team and schedule all necessary inspections in order to keep the work proceeding in a timely fashion. h. Except upon written instructions from the City, the Resident Engineer or Inspector shall not authorize any deviation from the Contract Documents. i. Determine if the project has been completed in accordance with the contract document and if the contractor has fulfilled all obligations. j. Periodically review the contractor's set of working drawings, updated regularly, to show final elevations and locations of all underground. k. Expedite acceptance of the project by maintaining a running punch list that informs the contractor of any deficient items and allows them time to correct deficiencies during the course of construction. I. Conduct a final inspection with the City and prepare a final punch list of corrective or incomplete work. m. Based on the Estimate of Time to construct all water, sewer, drainage, and roadway improvements, 110 calendar days of construction are anticipated for all off site and on site improvements, including all 3 phases. We propose to invoice only for the actual number of hours required, at the Hourly Billing Rate for each individual assigned to the Project based on a direct labor multiple (DLM) of 2.6. As a courtesy to the City, no direct vehicle or equipment will be billed to the project. The total CECS is $109,830.24 for all off site and on site improvements, including all 3 phases. TEG was hired by the Village of Fox Lake to Western Illinois University provide design and construction engineering services for the $510K reconstruction Macomb, IL of N. Frontage Road between IL Route 134 (Big Hollow Road) and approximately 150' Bachelor of Science, south of W Bayview Drive. The length of the project was approximately 1,950 feet, Construction Management or 0.37 miles. The work consisted of pavement removal, removal and disposal of unsuitable material, aggregate base course improvements, installation of combination SELECTED CONTINUING concrete curb and gutter, steel plate beam guardrail improvements, utility structure EDUCATION adjustments, rip rap outlets, signage installation, Thermoplastic pavement markings, IDOT QC/QA PROGRAM and final landscape restoration. TEG secured a Lake County permit for work at the Documentation of Contract intersection of Big Hollow Road, Lake County Highway A26. TEG also presented Quantities (#19-15511 exp. the project to the IDOT Detour Committee to obtain permission to utilize US-12 as 03/27/2023) the primary detour route. The project was constructed in a matter of three (3) stages in order to maintain access to the Fox Lake Crossing businesses. The project was SAFETY TRAINING completed under budget and on time. OSHA 30-hour Construction SafetyPHASE Ill, WARRENVILLE ROAD RECONSTRUCTION, CITY OF WARRENVILLE - Resident Engineer. TEG was hired by the City to provide planning, design, and construction Standard First Aid/CPR/AEDengineering services for this $1.3M MFT funded pavement reconstruction project. The project consisted of removing the existing permeable pavers, pervious base course and sub-base cross section and replacing it with a conventional full-depth HMA pavement for approximately 1 mile between IL Route 56 and Batavia Road. Due to excessive traffic speeds and pedestrian safety concerns, the project also featured traffic calming measures such as landscape median islands, lane narrowing, bike crossing pavement markings, LED stop signs, and stamped crosswalks. TEG provided a traffic study report which identified the needs for additional stop control along the corridor. PHASE Ill, INGENUITY WAY, CITY OF WEST CHICAGO - Resident Engineer. The City of West Chicago hired TEG to support the Development of Ingenuity Way owned by the DuPage Airport. Responsibilities include everything from engineering and inspection, contractor notifications, proof rolling and undercut inspections, asphalt inspections, sewer and water construction inspection, roadway construction inspection, lighting inspection, curb and gutter inspections, sidewalk and construction inspection. PHASE Ill, LANDON AVENUE SANITARY REPLACEMENT, CITY OF WARRENVILLE - Resident Engineer. TEG was hired by the City of Warrenville to provide planning, design, and construction engineering services for this $726,275 private sanitary sewer project. PHASE Ill, 1-355 VETERANS MEMORIAL MILE POST 0.0 TO MILE POST 12.3, ILLINOIS STATE TOLL HIGHWAY AUTHORITY - Construction Technician I. Contract was awarded just under $1.3. Scope consists of replacement of existing High Pressure Sodium (HPS) roadway lighting with Light Emitting Diodes (LED) luminaires. Inspection of existing lighting mass arms, replacement of deficient mast arms and installation of mast arms and installation of mast arm cable assemblies. Replace existing HPS underpass lighting with LED luminaires. Upgrade existing lighting controllers, and the associated maintenance of traffic. PHASE Ill, 1-88 REAGAN MEMORIAL CLEAN AND TELEVISE DRAINAGE SYSTEM, ILLINOIS STATE TOLL HIGHWAY AUTHORITY - Construction Technician I. The scope of work for this $1.3M project included cleaning and televising storm sewers, culverts and drainage structures, along with cleaning of ditches and erosion repair along 1-88 from West of Tollway Plaza 61 to East of IL 59. PHASE Ill, 1-88 REAGAN MEMORIAL ROADWAY RECONSTRUCTION MILE POST 138.1 TO MILE POST 140.5, ILLINOIS STATE TOLL HIGHWAY AUTHORITY - Construction Technician I. The awarded amount was over $44M. Rebuilding and widening the Reagan Memorial Tollway (1-88), between 1-290 and York Road in Oak Brook. The two-year project also includes rebuilding and widening the York Road Bridge, repairing and widening of the 1-88 bridges over Roosevelt Road, improvements on the ramps connecting 1-88 and the Tri-State Tollway (1-294) and noisewall construction adjacent to residential areas. PHASE Ill, 159TH STREET, HOMER GLEN, IL, ILLINOIS DEPARTMENT OF TRANSPORTATION - Project Engineer. The estimate project cost $55M. The proposed scope of work includes reconstruction and widening US Route 6 I IL Route 7 (159th Street) from Gougar Road Will-Cook Road. The improvement provides two lanes in each direction with variable width landscaped medians, and pavement bounded by combination curb and gutter on both sides and other related work. The length of the project is approximately 5 miles. The Project extends through two construction seasons. PHASE Ill, 159TH STREET, ORLAND PARK, IL, ILLINOIS DEPARTMENT OF TRANSPORTATION - Project Engineer. The project cost is $47M Reconstruction and added lanes, drainage improvements, shared-use path, sidewalks, and other related work on 2.53 miles of US Route 6 I IL 7 Route (159th Street) from Will-Cook Road to Ravinia Avenue located in Orland Park and Orlando Township. The project extends through two construction seasons. PHASE II, NAVY FEDERAL CREDIT UNION, CITY OF PENSACOLA, FLORIDA - Office Engineer. Contract was awarded for $400M. The 242-acre site is a parcel adjacent to the existing Navy Federal Credit Union campus in Pensacola, Florida. The Phase 2 Expansion and the scope of work includes two office buildings, an amenities building, two parking decks, equipment additions to the existing Central Energy Plant, and associated connecting bridges, roads, walking paths, stormwater retention, and infrastructure. The length of the project consists of 3 years. PHASE Ill, IL ROUTE 71 (ORCHARD RD-US34) OSWEGO, IL, ILLINOIS DEPARTMENT OF TRANSPORTATION DISTRICT 3 - Construction Technician I. The project consists of 2.62 miles of concrete highway reconstruction to widen its existing 2-lane road to 2-lanes in each direction on IL 71 from Minkler/Orchard Rd. to US 34 in Oswego, IL.
