ITEM SUMMARY:
Healthy West Chicago (HWC) was launched in 2014 as a collaborative community health initiative.
In 2015, HWC became a division of People Made Visible, Inc. (PMV), a 501(c)(3) nonprofit organiza-
tion. Under this structure, PMV serves as the fiscal agent, while programmatic direction is provided
through an Advisory Board representing local institutions, including the City, and other stakeholders.
The City has been a consistent financial supporter of HWC since its inception. Support has taken the
form of both base contributions and, in recent years, a matching component intended to encourage
investment by other taxing bodies.

2020: $85,000 total ($50,000 + $12,500 + $12,500 + $10,000)
2021: $37,500 total ($10,000 + $10,000 + $17,500)

2024: $20,000
2025: $20,000 requested (invoice received)
The outstanding invoice amount of $20,000 was budgeted for 2025. However, HWC was to present
an update to this Committee regarding its activities over the past year. Upon direction, staff will pro-
cess the invoice for an upcoming Disbursement Report.
Annual Report & Upcoming Request
An Annual Report for 2024 has been submitted and is attached for review. A representative from
HWC/PMV will attend the upcoming Committee meeting to present highlights of the past year and
request continued financial support.
For 2026, HWC is requesting funding not to exceed $30,000, which includes:
e $20,000 base contribution
e $10,000 match for partner contributions

other and with local institutions in ways that build trust, collaboration, and a stronger sense of
community?
e Elevating Culture & Community Identity
In what ways might HWC contribute to West Chicago’s shared identity—by supporting diver-
sity, belonging, and pride in the community—alongside other cultural and heritage initiatives?
e Investing in Our Future
As the City considers long-term investments in infrastructure, economic growth, and work-
force development, how might health and wellness programming fit into the vision of a more
resilient and prosperous community?
